Legal Consent Form Example: What a UAE Consent Letter Must Say
A legal consent form example shows how one person says yes in writing so another person can act, such as a parent letting a child travel or a sponsor allowing a visa. We have in-house, in person notary public services, online notary public services and home visit private notary services for it, and we draft the form for you.
Message us on WhatsApp +971 52 947 5935 with the document you need.
In short
- Price: a child travel consent letter is from AED 1,200 and a NOC for a visa is from AED 1,200.
- Who signs: the person giving consent, in front of a notary public.
- What to bring: the original ID of each signer, and details of the person and the purpose.
- Where: in person, on a home or hospital visit, or online through the official UAE channels.
- Next step: tell us the case and we draft the form.

What a Legal Consent Form Example Contains
Every consent form has the same few parts. If one is missing, the receiving school, airline, embassy or office may send it back. Acceptance is decided by the body that receives the document.
The full name and ID details of the person giving consent.
The full name of the person who receives the consent, or the child.
The exact act that is allowed, in plain words.
The dates or the period it covers, if it has one.
A line that the signer understands and agrees.
The date and signature.
No single legal consent form example fits every case, because a school letter and a travel letter ask for different details. Our free document templates page holds the wording you can adapt.
Ask about your document
Examples by Case
1
Child travel
One parent allows a child to travel with the other parent or an adult. The form names the child, the traveller, the trip and the dates. Read our guides on the child travel POA and consent letter and the consent letter from father to travel with mother.
2
Visa sponsor
A sponsor or parent says they do not object to a visa. See our page on the no objection letter for a visa.
3
School or teacher
A short letter lets a school or teacher act for your child, for example on a trip. Name the child, the event and the date, and keep it to one request. Whether a school asks for a notary public is for the school to say, so ask them first.
For the plain meaning of the term, see meaning of a consent letter.

Three Ways to Get It Notarised
Notarial acts in the UAE are regulated by Federal Decree-Law No. 20 of 2022. In Dubai, notaries public are regulated by Dubai Law No. 4 of 2013.
- In person with our in-house notary public. The notary public checks each signer's original ID, reads the document, watches you sign and applies the seal.
- Home or hospital visit. We come to your home, hospital, office or hotel, across all areas of Dubai.
- Online through the official UAE channels, the Ministry of Justice E-Notary System or Dubai Courts remote signing. We tell you whether your document can go this way.

We translate it with a legal translator if it is going to a body that needs another language.
You sign in front of the notary public.
If it is going abroad, we take care of the MOFAIC attestation.
Mistakes to avoid
Mistakes That Get It Refused
- A missing name or ID detail.
- Signing before the notary appointment.
- A vague act, such as "all matters".
