If one is missing, the receiving school, airline, embassy or office may send it back. Acceptance is decided by the body that receives the document. The full name and ID details of the person giving consent. The full name of the person who receives the consent, or the child. The exact act that is allowed, in plain words. The dates or the period it covers, if it has one. A line that the signer understands and agrees. The date and signature. Ask about your document ## Examples by Case 1 ### Child travel One parent allows a child to travel with the other parent or an adult. The form names the child, the traveller, the trip and the dates.
